| Salvatore "Gus" Yemma is an 82 year old gentleman who lives with his wife in a multi-level home. At home he was able to transfer independently and was independent with his ADL's and used a wheelchair for mobility. Gus had a fall while in his bathroom. This resulted in a fracture of his foot for which he was hospitalized. |

| In addition to his fractured foot, his diagnosis included paraparesis, A-fib, diabetes, hypertension and DVT and, while in the hospital, he developed a bedsore. His wife was unable to care for him at home because he was non-weight bearing on his foot and he was unable to transfer by himself. |

| Mr. Yemma spent two weeks at North East Rehab Hospital and was then admitted to The Oxford on November 8, 2010 for continuation of his rehab. He was initially able to transfer with the use of a slide board and one assist. Gus participated in an aggressive PT and OT program and progressed to the level of independent transfers and ADL's. A home evaluation was done prior to discharge to determine his safety in his own bathroom so further falls could be avoided. Mr. Yemma was discharged home with his wife and with community services on 12/21/10. It was the best Christmas present the Yemma family could have asked for. |
