| Mr. Sulesky is a very pleasant 92-year-old man who in his own home with his handicapped daughter. Carl is his daughter's primary caretaker. He went to the hospital on January 11, 2011 with difficulty walking and inability to take care of himself and with mental status changes. He was diagnosed with acute renal failure and delirium. |

| Carl came to The Oxford on January 21, 2011 for a comprehensive rehab program. He worked very hard with both physical and occupational therapy. He steadily progressed to the point where he was able to walk with a walker and required only minimal assistance with his bathing and dressing. |

| Carl was able to return to his home with his daughter and with community services on February 28, 2011. |
