| Myron Kalisz is a 98 year old man who lives alone in his own home. In mid October, he had a fall at home while retrieving his mail outside at the mailbox. He was admitted to the hospital on 10/8/10 with the diagnosis of foot fracture, rhabdomyolysis and hypothermia. His other diagnoses included anemia, macular degeneration, rheumatoid arthritis, hypertension and ischemic heart disease. Mr. Kalisz was admitted to The Oxford on 10/14/10 for a comprehensive rehab program in the hopes that he would be able to return home. Within a week of admission to The Oxford, Myron was re-admitted to the hospital with and MI and a GI bleed. When he returned to The Oxford on 10/21/10, he began his rehab program in earnest. |

| Myron vigorously participated in both PT and OT services and progressed rapidly. He was able to ambulate throughout the facility with the use of his rolling walker despite his impaired vision. He was able to perform his ADL's with set up and he was able to prepare light meals for himself. On 1/10/11, Myron was finally able to return to his home with his niece and with the help of community services. |
